The Nurse Educational Support Team (NEST) is a team of health care professionals that provide all Forsyth County moms that deliver at The Birth Center with an opportunity for a nurse visit, two weeks after delivery.
Home visits and/or virtual visits.
A Care Coordinator comes to you during your hospital stay with a bag of diapers and community resources to introduce our program and schedule an appointment. If we miss you, we will call you for an appointment.
During your nurse visit, home or virtual visit, we will provide breast feeding support, education about safe sleep, family planning, nutrition, newborn care, self-care and much more. The RN provides community resources and referrals to make connections to community partners. Available resources include food, clothing, diapers, support groups, mental health resources and additional programs that provide on-going postpartum supportive care.
In-person visits include baby weight check and BP check for moms.
Follow Up
Four weeks after the nurse visit, a team member will call to see how you are and if you need any additional support.
